NSAA Parent/Student Consent Form and Physical Information

Use the link below to print a copy of the NSAA Parent/Student Consent form.  Please submit this form along with your child's Physical form to the office prior to the first day of practice.  As a reminder, physical forms dated May 1st or later are acceptable for the following school year.  Any physical prior to May 1st will not be accepted according to NSAA guidelines.
